We will be happy to assist you with your insurance claim.
It is important to provide all insurance information and forms at the time of your visit. This includes current Medicare cards, insurance cards at the time of service. Please allow our checkout receptionist to review claim filing with you at the end of your exam.
If your insurance requires prior approval, it is the patients responsibility to contact your primary care physician prior to your appointment for authorization.
Some insurance companies require co-pay. This is due on the day of service.
Eye Care Specialists, P.S. is a medicare participating health care facility. Participation means that we accept fees approved by Medicare as our full fees. Medicare then pays 80% of the approved charges (less any deductible you have not paid for the year). The 20% left (plus any deductible you owe) is your responsibility. If you have other insurance coverage, your balance may be paid by that coverage. Surgical fees and arrangements will be discussed with each patient prior to surgery.
Please remember that your insurance coverage is a contract between you and your insurance company, not between the doctor and the insurance company. You are responsible for payment to the doctor.
If you are aware that your insurance company will not cover a visit or you do not have insurance coverage, we ask that you pay for each visit at the time you leave our office. Payment may be made by check, cash or bankcard. If there is a problem with payment in full on the day of service, please call our office to set up prior arrangements before your visit.
